WHAT ARE THE REQUIRED CLAIM DOCUMENTS?

Whenever there is a loss or damage or fire happening in your premises, you are advised to call 118 for fire incidents and/or our 24- hour hotline number 015 / 078 723 888 for further assistance.

 

In order for our claim personnel to act fast, please help to provide us the following information:

 

1.         Policy Number

2.        Your Company Name

3.         Time, Date and Place of Accident

4.        How the Accident Happened and Nature of Damage

 

You are obliged to minimize and prevent the extent of loss or damage and keep the concerned premises and lost or damaged properties intact for our claim personnel or appointed loss adjuster investigation.

The following documents are needed before we can process the claim payment for you:

 

1.         Fully completed Insurance Claim Form

2.        Police Report (if any)

3.         Fire Department Report (if it is a fire claim)

4.        Original Invoices or Receipts of loss or damaged items

5.        Inventory or Stock List at the time of loss

6.        Estimate Cost of Repair or Replacement

7.         Original Invoice or Receipt of Repair or Replacement

8.        Other documents reasonably required by Phillip Insurance
